Getting to Know Each Other

Letting someone into the home of an aging loved one is certainly not a decision to take lightly. We know you need a lot of information. The intake process is a little like dating. It’s a chance for us to get to know you, your family, your needs, and your expectations. But it’s also a chance for you to get to know us––learn more about our services, our approach, and even our values. Through this process you’ll get to know if Perfect Care Nursing is right for your family.

Step-by-Step Process

Schedule a formal assessment ($100 fee required, will be applied to services if hired).

Learn more about the client and their likes and dislikes.

Talk with the family and understand their expectations for the family member.

Discuss and develop a personal care plan for their loved one.

Assess the loved one and their environment to develop a care plan suitable for their environment.

Develop a weekly schedule based on the care needed and critical times.

Discuss agency policy and procedures, client’s rights and responsibilities, pandemic plan, and complaint information.

Upon agreement between both parties, first week deposit is due less the $100 assessment fee.
